Financial Policy

Thank you for choosing one of the North Cascade Family Physicians as your health care provider. We will do our best to provide you with the highest quality medical services. We feel that it is very important that our patients have a clear understanding of our expectations regarding your billing and payment. Please read and sign the following Financial Policy prior to your treatment. Should you have any questions, feel free to ask.
North Cascade Family Physicians is contracted with Aetna, DSHS, Group Health, Regence, Pacific Care, Premera, 1st Choice, Uniform Medical, Medicare, L & I, Molina and United Healthcare. For patients with these types of health coverage, we bill directly and accept their payment plus any co-payments, co-insurance, deductibles and payments for non-covered services owed by the patient as payment in full. If your policy has an office call co-payment, you must agree to pay the co-payment at the time of your visit. Failure to do so will result in an additional $15.00 fee. New patients without insurance will be required to pay the charge equivalent to procedure code 99202 on their first visit. Follow up visits for patients without insurance will require a minimum payment of $50 at the time of service.
If your insurance plan assigns you a Primary Care Physician (PCP) and it is not one of our physicians, you will not be seen.
For patients who are not insured through any of the above, we will be happy to courtesy bill your insurance company. Please provide our receptionist with the necessary information. These billings will be one time only and do not apply to any governmental plans. It will be your responsibility to follow-up with your insurance company should they deny payment for any reason. Keep in mind that you will receive statements from North Cascade Family Physicians until payment is received and that, ultimately, your account balance is your responsibility. NCFP does not bill insurance companies for motor vehicle accidents (MVAs). Charges related to care for an MVA will be billed on a private pay basis.
If you pay for your services by check and that check is returned for non-sufficient funds, it will immediately be turned over to a collection agency and your rights to continue as a patient at NCFP may be terminated.
Failure to make payment or contact with our Business Office within 60 days will begin the collection process on your account. If there is no response to these actions, we will refer your account for collection action. Repeated failure to pay may result in the unwillingness of all North Cascade Family Physicians to provide you and your family with medical services. Please contact our business office to set up a payment plan prior to your account going to collections.
Our providers are committed to providing the best treatment for our patients and our charges are based on a value scale developed by the American Medical Association and supported by most insurance companies. You are welcome to know what our normal charge is for any given services.
Showing up for your scheduled appointment time is very important. If you are unable to make your appointment, please give our office 2 hours notice so that we may give another patient that time. Patients that "no show" or cancel late for an appointment may be assessed an appointment charge ranging from $25 to $50. This charge is your responsibility; Insurance Companies do not pay for missed appointments. When a child of divorced parents is seen, we will expect payment from whichever parent accompanies the child. We will not bill ex-spouses or parents who live outside of the area. New patients that fail to show up for their first visit will be dismissed from the practice.
If you are having financial difficulty, our Business Office will be happy to work with you. You may want to establish a payment plan. If so, we ask that these payments be made as through automatic checking account withdrawals. We do monitor these accounts and non-payment may jeopardize your ability to be seen by our physicians.
